Bad CDR valve - broke the hose going into intake valley

So I determined my CDR valve on my 7.3 IDI is the reason why it's smoking randomly. It is very gunked up and sticking. CDR valve is basically a PCV valve for a diesel. It does pretty much the same thing, except it operates somewhat differently. Anywho, the hose that goes into the intake valley end broke off and I had to cut out what was left, including the grommet. Is there a replacement for this plastic hose and grommet, and if so does anyone know the part number off hand? I have the part number for the CDR valve itself.

My other option would be just to run a road draft tube and delete the CDR since it's not really necessary, but I'd rather not.
